Optional Life and AD&D

In addition to Basic Life and AD&D, you may purchase additional Voluntary Life and AD&D insurance at favorable group rates. You pay 100% of the premium. Note: It is important to provide beneficiary information during the enrollment process and keep this information up to date. If you decline Voluntary Life insurance when first eligible or if you elect coverage and wish to increase your benefit amount at a later date, Evidence of Insurability (EOI) may be required before coverage is approved.

Voluntary Life insurance – You may elect additional coverage for yourself in increments of $10,000 up to a maximum of $500,000. You may elect up to $500,000 of coverage for yourself when first eligible without providing EOI.

If you enroll yourself in Voluntary Life coverage, you may also elect coverage for your spouse in increments of $10,000 up to a maximum of $500,000. You may elect up to $50,000 of coverage for your spouse when first eligible without providing EOI. The spouse coverage can be up to 100% of the employee’s coverage. Spouse coverage may not exceed employee’s coverage amount.

If you enroll yourself in Voluntary Life coverage, you may also elect coverage for your child/children in increments of $5,000 up to a maximum of $10,000.

Voluntary AD&D – You may elect additional coverage for yourself and your spouse in increments of $10,000 up to a maximum of $500,000. If you enroll yourself in coverage, you may elect coverage for your eligible children in a flat amount of $5,000 or $10,000.

Guaranteed Issue and Evidence of Insurability

Employees and spouses who elect Voluntary Life and AD&D coverage when they are first eligible can elect up to the Guaranteed Issue (GI) amount without Evidence of Insurability (EOI). If the amount requested is more than GI, you will need to provide EOI before the amount over GI becomes effective.
